Please allow at least 1 hour for your first visit.  A chiropractic examination will be the most thorough exam you may have ever experienced!  Because we deal with eliminating the cause of your symptoms, we want to know absolutely everything about you!

If you are taking prescribed or over-the-counter medications, supplements, or vitamins - please bring either the bottles or a list of the name, dosage, and frequency of each.

While not necessary, some of our patients feel more comfortable bringing a pair of shorts in for the exam.  This will allow the Doctor to fully and accurately assess your ranges of motion, so that they may provide you with the most applicable treatment plan.

If you choose to print off the new patient paperwork from our website, please also bring in any applicable insurance cards, as it takes about 24 hours for us to receive your benefit package information back from most of our insurance companies.
